Adoptees and adoptable non-adopted persons who made a request for and received services since June 16, 2018 and who now wish to submit a new request for services within the framework of the provisions of the Act respecting family law reform with regard to filiation and amending the Civil Code in relation to personality rights and civil status can make a simplified request. To make such a request, here is the form to complete.
