This booklet is an important document. It is the only record of all the vaccines you have received. You will need it all your life, so keep it in a safe place and keep it up to date. Take it with you to each medical appointment !
IMPORTANT : Please note that an error has occurred on page 8 of the document. A dose of the meningococcus C vaccine is included in the regular vaccination schedule at 18 months rather than 12 months of age. For an up-to-date version of the regular immunization schedule, see the Immunization Schedule for children born as from June 1, 2019 information sheet.
During a Covid-19 vaccination appointment, proof of vaccination will be given to the vaccinated person.
